Key Responsibilities

Seeking a Social Worker for our practice who provides clinical expertise to ensure all patients receive high quality, efficient care. We are an amazing team that works hard to support each other and are seeking a phenomenal addition like you who feels patient care is as meaningful as we do.

Skills, Experiences

The Social Worker:

  • Provides direct social service and counseling to patients/families
  • Assists in locating appropriate resources for post-discharge care
  • Participate in multidisciplinary team to coordinate the client’s plan
  • Cross-trains with team members to serve as back up resource
  • Assesses and identifies cases for intervention
  • Prioritizes goals and develops plans based on client needs
  • Monitors and evaluates outcome of client’s plan
  • Proactively identifies cases
  • Collaborates with third party payors internal case management/disease management departments as appropriate for discharge planning needs


Education: 

  • Bachelors degree or Masters degree required
